What is the passport?
The healthcare passport is a document that you can fill out and keep. You can then bring it with you to any healthcare appointments and ask your healthcare workers to read it before interacting with you.
What does the passport do?
The passport helps to support your self-advocacy by outlining your healthcare needs, including your communication and sensory needs.
Who created it?
This passport has been designed by the team at Thriving Autistic, inspired by both the HSE Health Passport for people with intellectual difficulties and the UK’s National Autistic Society Health Passport.
